  • Exposure to ambient air pollution increases mortality and is a leading contributor to the global disease burden [1]. Epidemiological studies have elucidated a relationship between out-of-hospital cardiac arrests (OHCAs) and air pollutants, especially particulate matter (diameter ≤2.5 μm; PM2.5) [2, 3]. The causes of OHCA are broadly categorised as cardiac and non-cardiac [4]. A 10 µg·m−3 increase in PM2.5 exposure yielded a 1.6% increase in the incidence of cardiac origin OHCA [3, 5]. However, few studies on OHCAs of non-cardiac origin, including intrinsic respiratory diseases (COPD/pneumonia/asthma) are available. We examined the association between short-term exposure to PM2.5 and bystander-witnessed respiratory origin OHCAs, including eventual prognosis. We also investigated differences between PM2.5 exposure-related cardiac and respiratory origin OHCAs. Particulate matter is a potential risk factor for out-of-hospital cardiac arrests (OHCAs) of respiratory origin. However predictors of SCA are still unclear. Aims and objectives. The aim of the study was the analysis of SCA frequency in severe respiratory infection and studying of factors associated with it. Methods. 55 pts with severe respiratory infections in ICU and respiratory department were examined. Respiratory infections (severe pneumonias) were verified on the basis of existence of lung infiltrate according on a chest X-ray or CT-scans. All of the patients recieved additional oxygen or respiratory support, antibiotics and fluid resuscitation. Multifactorial regression analysis and the relative risk indicator were calculated. Results. Despite a carried-out intensive care 22 pts (40%) died. The sudden cardiac arrest occurred in 8 pts (36,3%), not because of septic shock or multiorgan dysfunction. All of patients at the time of SCA were in full consciousness, and the probable cause of SCA was the cardiac arrhythmia.   • Aim: We have shown that Azithromycin (AZM) blocks airway epithelial cell autophagy. This is contrary to autophagy and AZM’s role of clearing intracellular microbes, and hence may promote bacterial residence and tolerance. Hypothesis: Altering the charge state of AZM or cotreatment with promoters of autophagy counters AZM’s off-target block of autophagy. Method: The 16HBE14o- line was used to model/assess autophagy. Synthetic chemistry was used to produce an oxidised form of AZM, termed Δ-AZM. In parallel experiments, the autophagy promoters Ku-0063794 (KU) or Torin-1 were co-treated with conventional AZM. Structure-activity analyses were then performed by quantifying the abundance of LC3-II (indispensable for autophagy) vs Sequestosome (decreases when autophagy is promoted), to generate a readout for autophagic-flux using Western blot analysis. Results: KU (10 uM) or Torin-1 (5-10 nM) significantly released AZM-based (50 ug/mL) block of autophagy ( P<0.01 ). 9a-NBD-AZM (a fluorescent AZM analogue) loaded into 16HBE14o- cells exposed to KU, showed this was achieved by the cells synthesising further autophagosomes. Δ-AZM significantly mitigated blockade of autophagic flux vs conventional AZM ( P<0.001 ). However, Δ-AZM exhibited a lower bacteriostatic effect in an E coli model and reduced anti-inflammatory properties, as determined using LPS stimulation and readouts for IL-6.   • Background: Limbs pneumatic compression during cardiopulmonary resuscitation for cardiac arrest patient were rarely reported. We aimed to evaluate the impact of this therapy on the patients with cardiac arrest. Methods: A prospective and controlled study, a total of 46 elderly(74.8±5.3 yrs) cardiac arrest patients were included and randomized to “CPR group “(n=23) and “CPR + Pneumatic Compression group” (n=23). Each in CPR group accepted standard CPR. In addition to standard CPR, all subjects in “Pneumatic Compression group” received intermittent pneumatic compression(IPC) by Air Compression Therapy System, which digital six-tiered rippling full-body pressure (0.45~0.6kg/cm2) and 1 minute interval every 3 minutes. Results: Patients with standard CPR had higher incidence of acute kidney injury than in “CPR + Pneumatic Compression group” (52.2% vs. 21.7%, p =0.032). The lower occurrence of hypoxic ischemic encephalopathy and significant improvements of survival were recorded in Pneumatic Compression group than in CPR group(Figure 1,Figure 2).
